Overview

Once you setup your contact form (this should be done before you start this tutorial), you can add custom profile fields to that form.  I will take you through 2 easy steps in this process using a fairly common form field: "How did you hear about us?".   This can be done to add whatever field you would like to your custom contact form.

1 - Create the Custom Field.

So start off by going to Settings in the left navigation.  This will bring up your Account Settings that has a ton of different items you can customize.  One of those options in the middle blue module under the Customizations heading is Create custom profile fields.  Click on that and it takes you to the Custom Profile Fields Editor.

Now, click the New Custom Profile Field button at the top.   And you can fill the information in similar to this below:

Once you click save, you have created a custom profile field that is a drop down with the options mentioned, attached to clients (so you can see it in the Client Editor), and viewable in Client Access.  If you do not want it viewable in Client Access, just uncheck the check box.

2 - Add it to your Contact Form.

Now that we have created this custom profile field, go to your Contact Form and you will see that it is at the bottom of the fields to include and defaulted to "hidden".   You will see that I changed it to optional below.  You can make this field optional or required and it will show up on your contact form for your clients to fill out.  You can also rearrange the order of the fields by using drag and drop into the locations you desire.
